The ripple effect of healthcare reform is beginning to impact care delivery strategies as care management now falls increasingly to providers.
According to a recent HealthLeaders Intelligence survey, hospital leaders are making progress with care management efforts, but more robust tools will be needed if hospitals want to scale up. The October 2014 survey polled 134 senior, clinical, operations, finance, marketing, and information leaders across the healthcare spectrum. The majority of respondents were from nonprofit organizations (63%), while the remainder (37%) came from for-profit settings.

That pivotal experience, while traumatic, solidified friendships with his daughter’s nurses and brought him several years later to Planetree, a nonprofit organization started in 1978 by a patient who wanted to help hospitals deliver stronger patient-centered care practices. Planetree works with 700 organizations in more than 17 countries.
Providers are increasingly making the leap and investing in their organizations in preparation for value-based care. However, while no one wants to be behind the competency curve when it arrives, it can be expensive to build competency for a new model before it is financially viable, causing providers to remain cautious.

If you’re not one of those retailers, you’re likely busy playing catch-up to them – and that’s not a position in which anyone wants to be.
Since eCommerce began, a small handful of leaders have set the pace. They’ve innovated quickly and delivered incredibly valuable and convenient shopping experiences. And in doing so, they shaped consumer expectations.
Take Amazon. Over a decade ago, the online giant seemingly rewrote the natural laws of retail by managing to deliver more, better, faster – all with unmatched accuracy and ease. Now, consumers expect an Amazon-like experience everywhere they shop, and anything else feels subpar.
Omnichannel is one of those words everyone
uses, but few use correctly. It’s not another word for
multichannel. It’s a different state of existence.
In our experience, multichannel is the ability to interact
with customers across many different channels, but
not necessarily in a cohesive fashion. As new channels
become available, they are “bolted on” to existing
customer experience infrastructure such as CRM
or customer support systems. Too frequently, the
management of these channels becomes siloed (web
versus in-store sales, for example). The effect of this from
the customer experience standpoint can be disjointed
and jarring. For example, the resolution of an issue
following a customer conversation with an associate
in the contact center may not be reflected when the
customer goes online or into a store.
Yet the ability to offer multichannel service experience is
